Named Accountable GP For All Patients

The 2014 -2015 GP Contract required all GP Practices to allocate a named accountable GP to all patients aged 75 years and above.

The 2015 - 2016 has further built on this requirement with all patients, including children to be allocated a named accountable GP by 31 March 2016.

Appleton Village Surgery have allocated all patients a named accountable GP.  The named accountable GP is responsible for the co-ordination of all appropriate services required under the contract ensuring they are delivered to each patient where required (based on the clinical judgement of the named accountable GP).

Although each patient has been allocated an accountable named GP patients are free to consult with any GP/clinician of their choice.
