Relocation News

Have you heard about our proposals to relocate the surgery to a brand new, purpose-built site?

The move has received approval, however we are experiencing a delay due to issues with Halton Borough Council. As soon as further news is available, we will update this site. 

Appleton Village Surgery Consultation Presentation

Christmas Opening Hours at Appleton Village Surgery.

Monday 24th December 2018 -  Normal surgery opening hours (7.00am - 6.30pm)

Tuesday 25th December 2018 - The surgery is closed.

Wednesday 26th December 2018 - The surgery is closed.

Thursday 27th December 2018 - The surgery will re-open as normal 8.30am - 6.30pm. 

Tuesday 1st January 2019 -  The surgery is closed.

Throughout the Christmas period both The Urgent Care Centre and Out of Hours will be open as normal.

Friends & Family Test

The NHS Friends & Family Test (FFT) is an important opportunity for patients to feedback on the care and treatment received from your GP Practices.  Have your say by completing the Friend & Family survey either by completing the on-line survey or by completing a Friend & Family survey in the Practice.

Closed for Protected Learning - Thursday 25 October 2018.

The surgery is closed from 12.30 pm on Thursday 25th October 2018 for protected staff training.  If you have a medical emergency that will not safely wait until we reopen the following day, then call NHS 111 by telephoning 111.

Patient On-Line Access

By registering for 'Patient On-Line Access' you will be able to book and/or cancel an appointment with a clinician and order repeat prescriptions.  You will also be able to access area's of your medical records such as allergies, test results, immunisation history and coded information (only available from 31 March 2016).  Access to this service must be made by completing the relevant registration form and providing proof of identification.  For further information please see a member of the Practices Reception Team
